More space/sections on the add disc id page

looking at http://test.musicbrainz.org/cdtoc/attach?toc=1+3+60990+150+20282+40957&artist=615863 it's quite confusing and could use more whitespace around each release. I think now we have release groups, we could use the release group names as headings to help split it up

MBS-684 has been fixed so this page looks much nicer now.

Sorting by release group sounds even nicer, but I'll let someone else do that.

nikki: I'm wondering what you think of this since the MBS-684 changes – should it still be grouped by release group?
